It seems nearly impossible to play this one on realistic mode.
Here is what I've encountered:

- Brakes are nearly unusable. When going down any decline, you must use full brakes and even then they are barely able to keep you from speeding up. Once the decline is over 2%, even using emergency brakes can't keep the speed from increasing.
- Throttle position. If you're at 5% throttle and press S to decrease it more than once, the throttle will go negative, all the way to -100% if you keep pressing it.
- Reverser. Not sure if this is intended or not but if you're moving forwards and have zero throttle and change the reverser to anything negative, it will show wheel slippages and the speedometer will jump all over the place. I tried using reverse on declines where the train is unstoppable even with emergency brakes on but it makes the wheels slip and the generally makes the AI take over and hit the emergency brake which mysteriously stops the train fast even tho I was already in full emergency brakes.
- Reverser and throttle. Noticed several times when not moving when I had the throttle at zero and I moved the reverser to something above 0% suddenly I'm moving forward and it says the wheels are slipping.
 
These issues are all related to the loco and the consist - nothing to do with the route itself. If you are having problems driving a particular loco then you need to indicate which loco it is and how you have configured the consist.
 
I played C&O 2-6-6-6 H8 - New River Mining Coal Run and Thursmond Empties. I played the loco as the game had it setup, I didn't change anything.

Just now I loaded my save game where I was going down a -3.9% grade at 2 MPH with brakes on full. Even after engaging emergency brakes it continued to accelerate to 6 MPH until the grade got to around -3% then it finally was able to stop.

Also I did some testing on a -0.5% grade. I came to a stop then with the regulator at 0%, I changed the reverser to -75% and the train starting moving backwards, up the hill. Surely it shouldn't have any power with 0% regulator right? Also if the regulator is at 4% or lower, pressing S puts it negative, all the way up to -100% if you keep pressing it, but it didn't seem to make a difference from 0%.
