As I understand it, whenever the CM database contains an asset that has either a newer version of that kuid (ie, in kuid2 format the last digit is higher than the current), or an asset that specifies that asset in it's obsolete-table.

Note that the CM database isn't 'live', it is updated regularly, but may lag behind a few days.

Broken 'updates' can occur, I believe, when an asset is obsoleted via the obsolete-table, but then a later version of that asset removes the original from the obsolete-table, in this situation the original asset is marked 'update available' yet no update will be locatable (since it's hard to find the updated asset manually, this can be hard to find, although the 'content updates' page in ts2010 appears to track updates that occur due to obsolete-table correctly)

For assets that have an Update available Status icon showing in CM3.2, right click it ans select View Asset Versions then click the View in main list. Download the kuid with the largest version number.
