Here is a few pics of a rare malfunction. Though the body is not shown, this happened on a BNSF Loco My Navy friend posted to his FB page.

I my self do not have any real info on this. I aquired this from a FB photo with no added details. As for speed. I can only guess not really fast as this is at a fuel point for locos. (This is my friends post navy career, A loco fueler/sander as he posted.) The fact a piece of bogie housing is found near by also adds to the reason I believe it happened on a slow speed zone of a yard or siding.
 
If that would have happened at speed it wouldnt have been one hell of a ride! Not to mention if you kicked in the brakes the shoe would catch and break even more stuff.

More than likely at speed, a broken off 1/3 wheel fracture would have kicked in some sort of shutdown of the consist, stopping the train when the first break hose popped, while the unit was being dragged.

this makes no sense at all.

more than likely nobody would notice immediately if the wheel broke. out of 12 wheels 1 breaking isn't going to make much of a difference. an alarm possibly sounded when the gear case came apart (thats the thing in the 3rd image).
