Almost no system can function without some human intervention, and where there is human intervention there is error. Someone was distracted or tired or drunk and did something dumb. It cost lives. Sad, but it's the human condition.
This is reminds me of the train/bus crash in the NW Chicago burbs where
some years ago the bus (school I think) didn't clear the tracks at Hwy 14 and was hit by a Metra train. I wonder if the cause was the same in this case.

Good point. There is now a small memorial located there next to the ROW. The timing of the traffic signals was lengthed to allow traffic to get off the crossing when the warning bells are activated. The school bus driver thought she was clear of the tracks, but was not. Still; if the signal changes and a driver does not notice, or is slow in responding, a similar accident could occur even now.
