One could make the obvious observation here. Microsoft has the resources, $$$$, programmers, analysts, designers, alpha (i.e. "in house") testers, etc and yet they still cannot get it right. Clearly they either did not test enough (and how much testing is actually enough?) or their in house systems do not fully match the range and variety of system that exist "in the wild". Other system and software developers would have similar stories.
