I happens every now and then. It's often the cause of trains getting stuck at a red signal near a junction forever. The driver cannot reach two levers down the line, generally a single track signal block, to correct the wrong setting at the far end.

You need a signal between the 2 junctions, that way he will pass the first one, which will be yellow, then switch the second one to where it needs to be.

If I do that, two oncoming AI-driven trains will get in each other's way. I've tried putting signals in between junctions like this before, and two opposing trains came face to face and hung up. No more than one rail vehicle is allowed to occupy that track section between the switches. This is why switches on the far ends need to be set to diverge right by default and AI needs to put them back after passing them. The approaching train on the near end needs a clear signal path down the line past both turnouts. This train can control the nearest lever unless an oncoming train locks it.

Did you try the "Control Junction" diver command? Make sure the driver also frees the junction after he has gone through it.
