I took a quick look at the study last night done on the 2010 crash in Utah between Amtrak No. 5 and a truck. Sadly this caused the death of 5 people including the driver. The investigation looked at everything including the crash safety of the train cars, the cell phone records of the driver, etc. Of particular interest to me was the interviews with the conductors and the engineer.
My observervations and conclusion regarding this accident are that truck driver was not paying attention and "woke" up too late to stop safely. He was on his cell phone at the time so perhaps that contributed to his failure to stop. There are warning light placed far back from the crossing to warn motorists of the crossing, but he and may others probably don't pay attention to them.
