According to the investigation, the bridge had problems prior and it reported failures. When this train was to cross the bridge, the bridge failed to change the signal to green, and the crew received permission from the dispatcher to cross anyway. IMHO the bridge, though having mechanical failures, it was reporting that it did have problems. It's like smelling gasoline and your car doesn't start. The fact that a train that past over the bridge, the night before, received an error on the bridge opening, indicated something was wrong. The other fact that the bridge was acting odd by being closed, but still showing red. Why did the dispatcher authorize the crew to cross anyway? The investigation is continuing and there will be more videos as time goes on. I'm not blaming any one person in particular, but the failure of a system that was put in place to safeguard a problem such as this.
