For whatever reason when I place crossovers only two switches are placed instead of four. How can I fix this issue? These are the PBR junction levers.

I just tried it in TRS22PE and when you add a crossover between two parallel tracks only the second junction gets a switch.

Repeated the exercise in Trainz Plus (126280) with the same track and the same result.
Repeated again in Trainz Plus (beta 127161) with the same track and there was no problem. Levers attached to all your track junctions.

I would say that this is one of the known bugs (but one that I have never noticed before) that have been fixed in the latest beta.

I have this happening too.
Until the next retail update (SP5??) is released the simple solution is to manually add the missing levers after creating a turnout junction. I always replace the default lever with my own preferred levers, of several different types including invisible levers, so it has never been a "show stopper" for me.

Weird....I was just going to post this same question! Never had this problem until recently.
Same here. I've had that happen occasionally in the past and chocked this up to my doings. If this has been fixed, then this was one of the many not-mentioned bugs that have plagued the last version. I replace the default levers immediately anyway after laying track, so it's no big deal to check for a missing lever but it's good to be aware of the situation.
