CSX Derailed... AGAIN.

Yesterday, a CSX Intermodel hit a local up near Syracuse with CSX 8320.
A YN1 LOCO.
Luckily, only 1 person was injured.

Any word on the cause of this derailment?

Off topic: My uncle lived in Johnson City right on Endicott Street. There was a railroad bridge right over the street across from his house. When I visited him, I sat on the porch for hours watching the old Erie-Lackawanna freights go by.

Apparently, both trains had Green upon there signals, but it was read wrong,
someone thought it was a medium approach, but it was really restricted.
The trains hit at 30 mph.
Also Off Topic: So you mean the old power plant branch, Conrail scraped the line right after NPP(National Pipe & Plastic). In the spring, Johnson City closed the plant right up.
