The usual method is to create a station and give it the name using the ? button in Surveyor. Industries can also be given names in the same way. AFAIK towns as a whole can't be given names. However, you can add city limit signs with names on roads, plus signposts pointing towards the town.

There is a built-in (to 2004 anyway) called "Sign Station" (KUID -1:101440) that is just a simple crosspost white sign. You can put text on it - and even long texts (though it will extend beyond the sign itself) using the "?" button. We use this in the DHR a lot. You can "hide" it inside a building so it won't be seen, but the minimap will show it clearly.
